Student and Parent need to attend-Bring- 2 proofs of residency, free and reduced lunch form or be willing to pay the $45 form that night

You Must Turn in TWO PROOFS of RESIDENCE to play sports!

Utility Bill, Electric Bill, Mortgage Statement, Water/Sewer Bill, Natural Gas Bill, Parent’s Driver License, Voter’s Registration Card, Letter from employer on company letterhead stating that you live at that address

NOT ACCEPTABLE: Any other bill, telephone/cable bill, credit card bill, student’s driver license

Please note: All proofs must be in parents names and must be where you live with your parents

Reminders- no paperwork, no play…..no physical/concussion form, no play, no GCS Fee Waiver Form……no play.
